System Update 3.14 fails on final last step and rolls back

I am attempting to install SU 3.14 on my fathers T30 2366-8AU but the software will not stick.  It goes through the entire installation process then at the end when it appears to be completing it "rolls back" and uninstalls.

 

How do I isolate the problem that is causing this here?
