stylus is stuck and button popped of

First the button popped off. I found it, replaced it.  It seemed to work but popped off again,   Now I can't find it.  I returned the stylus to its home and cannot get it to come out.  I cannot get a grip on it with Vice-Grips (it is very slightly out, but not enough).  How do I get it out, and why did the button pop off?

The stylus was purchased October  2008.

I opened the Tablet to remove the stylus, but I still do not know why the "right-click" button came off or how to repair it.   It is kind of pricey for that kind of performance.  Any help here?
