I dont want to mess with voltages etc. yet, so I ordered this - https://www.coolermaster.com/catalog/coolers/notebook-cooler/notepal-u3-plus/ I know it probably wont do much, but I wanted a stand anyway and liked this one. I should have it next week. I will do some testing after that and let you know if it makes any difference with and without the fans.
Edit: On which side of the Y540 is the processor? If you look at your screen, left or right?

Does anyone really know what excatly does those Vantage heat profiles really do?
Have you tried gaming with Silent mode? Since I think that it does somewhat the same thing than undervolting or reducing wattage does. I replied to the Y540 topic already, so I won't repeat the whole thing, but some notes from my tests with Doom.
Moving from Performance or Balanced mode to Silent mode does not affect on game FPS that much. Laptop is a bit more silent. Temperatures go way down. I have a stock machine with stock settings (no undervolting or anything).
- Max heat drops about 12c° from 93c° to 81c°.
- Wattage drops from about 74W down to about 51W (that is the max I have seen on Silent).
- Voltage may not change at all. There is a minor difference, like 0,019V though. 1.358V to 1.339V.
- CPU max GHz does not change or if changes, changes very little. Maybe 100Mhz.
- Doom performance drops from steady 144fps to something like 100-144fps.
So I think Silent mode is ideal for gaming and you dont have to do any changes like undervolting. Am I missing something important here? Because it feels strange if the solution is this simple. For now I mean. I do hope they give back max fan at some update, because more power may be needed on heavier games.

Here, see these:
They are a bit long but this guy is very thorough, this should tell everything you need to know, he even mentions this thread...

Yup silent mode diables the turbo boost of cpu leading to 2.5Ghz clocks and nice drop in temperatures without undervolting and there is not much loss of fps too in games
