First of all your laptop is not really designed for gaming, it's more on the entertainment and workstation purposes.
Your GPU however can play some demanding games at reasonable settings. Since it's Nvidia and is powerful it's going to be power hungry and will make your fans speed high.
It appears your laptop has been reducing the power of your GPU (and even CPU) so that it doesn't heat up too much. What games do you play? And how old is your laptop? Maybe the fan is dying and needs some repair or replacement. Or something else in your motherboard.
And don't game too much as well, since again your laptop isn't design for gaming. It has inadequate cooling so it's gonna heat up. It may need also a bit more rest this time.
