Only the dedictaed GPU will be used during gaming (860M)
This card is good enough to play most new games at med/high settings?
What kind of problems are you facing?
Which game and what settings are you playing on?
Have you enabled Tessellation, Ambient Occlusion or MSAA in the game?

One thing you can do to ensure highest perfomance on games is go to the Nvidia Control Panel, Manage 3D Settings, Programs, then find the game you want and set it to use only NVIDIA High Performance (or Discrete) graphics.
You can't use both cards at the same time as of writing this (DirectX 11), but with the arrival of Windows 10 and DirectX 12 on July 29 you will be able to use both at once to increase performance. Until then, though, it is simply not possible to use both at once. The best option is to just use the GTX 860M.

Well, 860M is a medium level card, and it should be able to run most games at med/high settings and 1080p resolution.
If you try to push the game graphics with Tessellation, Ambient Occlusion then your FPS will come down drastically.
As ArchLinux mentioned you cannot use both the cards in DX11, wait for DX12.
I am not sure which games you are trying, at what settings and what is the fps???
Without this data it is very difficult to judge your scenario.
