First, if you're not comfortable with opening up the laptop, I would suggest contacting Lenovo support directly for a repair. Their number is 1-877-453-6686. And if you're under warranty, they'll cover it.
But if you want to do it yourself, you'll need to replace the IO board: https://www.ebay.com/itm/OEM-Lenovo-Yoga-3-Pro-1370-Laptop-USB-MICRO-HDMI-PORT-BOARD-NS-A321/123252539901?epid=1739872396&hash=item1cb26c81fd:g:IJ4AAOSwUyBbS8jT
You'll need to take out the ribbon cable from the broken IO board and swap it onto the new IO board, then connect it to the motherboard. You'll also need a T5 Torx screwdriver to open the case itself. You don't really need any other tools, but using a spudger would be helpful for removing the ribbon cable, here's a whole set that's pretty inexpensive: https://www.amazon.com/Zacro-Professional-Non-Abrasive-Spudgers-Anti-Static/dp/B01F52ZPBQ/ref=sr_1_1_sspa?ie=UTF8&qid=1537359182&sr=8-1-spons&keywords=spudger&psc=1
Lenovo's HMM has good information on how to get into your computer (HDMI board is on pg 31): https://download.lenovo.com/consumer/mobiles_pub/lenovo_yoga_3_pro_1370_hmm_201512.pdf
Hope that helps!
