Appreciate your post and the steps you've tried. Have you tried booting the phone in safemode and see if it will occur there.
To Enable Safe Mode
1. Press and hold the Power button until Power off comes up on the screen.
2. Touch and HOLD Power off on the screen until reboot to safe mode appears.
3. Touch OK

Had the same difficulty when upgrading to 64GB sdcard.
When I reformated the sdcard from internal memory to extra memory the issue was resolved.
Not sure if this would work for everyone, but it did for me.

I have reviewed the comments and feedback to previous posts and my problem still persists. I have not been able to use my camera for 2 weeks now. I removed the faulty SD card and it still persists. Kindly help

Thanks for posting. I'd like to help. Did you restore your apps/files from your old device to your new phone? If so, try doing a factory data reset without restoring your apps. Backup your important files so you won't lose any of them. Once the phone is reset, test the camera and install your apps one at a time until we find which app could cause this.

Thanks Rich, yes I tried absutley everything and every combination, in safe mode from a factoy reset and still nothing. I am trying to return the phone and get a refund. The process is very complicated and long, I don't think I will use any of Lenovo/Motorola products and services again. But thanks for trying to help.
