I'd insert it while the phone is off and then turn it on.
When it comes to removing it, you should probably turn the phone off, and then remove it. At the very least, you should unmount it before removing it.

yes I found various cards which worked elsewhere were just constantly unmounted automatically from my 2014 moto g... eventually the one that worked was a brand new Kingston 32gb micro SD HC. good luck!!

I am having the same problem of this thread with a moto g LTE (since in 2014 as far I know there wasn't a 2nd Gen of this phone I think this is the correct thread to use) + android 4.4.4 + kingston 16gb class 10 card.
It worked only for a few minutes, then I tried the to format the sdcard in FAT32 and only worked until I rebooted the phone. Also tried factory reset of the phone. It worked fine with a 2gb Sandisk card. And the sdcard worked correctly in other phone and in a pc.

I have a just buyed XT1040, i purchased a Verbatim microSD HC 16Gb Class 10 and the phone ignores it !
Have tried reformatting to FAT but still isnt working.
My old micro SD 4Gb class 4 works fine.
I have purchased the micro SD according to what the phone specs say, there is a fix ?
How can i buy a new SD and be sure its going to work fine ? all come in closed blisters...
