Check the I/O Access settings in the BIOS. look to see if "Card Reader" is disabled. That would prevent it from appearing in the OS.
The i/o access settings should be under The security heading in the BIOS settings.

Everything under I/O Port Access is Enabled. Nothing that is "Card Reader" named specific. There is Memory Card Slog and ExpressCard Slot and Integrated Camera that are the only ones the possibly represent the Card Reader.......

It would be memory card slot.
This is interesting.
So it just dissappeared after a BIOS and or driver update, and won't read cards?
I know on my W530, it doesnt appear unless a card is in it, but it sounds more like some update clobbered it.
EDIT: does it appear in device manager?

Yeah, I feel like it was completely clobbered and installing the driver just is doing nothing.
If I'm in Device Manager, where should it appear?

On my laptop It shows under Memory Technology Driver. But mine has a JMicron SD Reader. I feel that it should be a similar category for the O2/Realtek readers. If you can't find it, see if you can grab and upload a screenshot of your device manager.

intriguing. since its completely gone and updates don't seem to fix it, calling lenovo might be the only option to get it fixed.
One last option I can think of is rolling back the driver/BIOS update and seeing if that fixes the issue.

Yes - think the reader is simply no good. We are testing another W540 that came in at the same time so we'll just send this back.
