This particular update (KB4093119) keeps failing. Last time waited 4-5 hours, and I had to hit the power button, and restore the system. The screen below appears and there is not screen activity, no HD light activity, nothing, just sits there. I am able to run other programs, and other tasks. It stays at 100% from the start, with no status indicator.
Is KB4093119 (2018-04) a know issue on the W540 with Win 10 1607??
Any way for me to know if the computer is downloading the update, is stuck, or in a loop???? ^AltDel does not seem to reveal any d/l activity.

I had a similar problem on a ThinkPad T440p (same generation, different model line). It turned out the OS volume was down to about 5GB free, and the update didn't like this. Freeing up an additional ~10GB of disk space allowed the update to install as normal.
I am not sure if this will apply to Windows 10 Pro running on a ThinkPad W540, though, but perhaps it will help.

It has been reported (on some forums) that the 1803 update creates a new partition on the boot drive. On my P70, that partition is 881MB. You will need at least this much available space to accomodate this.

Also, be aware that drivers for some features are not readily available yet. NVIDIA (display/monitor/GPU) and SYNAPTICS (mouse/fingerprint) are among these at this posting. (There are probably others too.)

As of today, February 1, 2019, my fantastic workstation W540, is stuck with Windows 10 pro 1607.14393.2214, and will not update. MS tech support worked on the issue for a month. Their simple solution after 30+ days, just reinstall Win 10 and all the software , and of course call all licensing vendors (eg: Adobe, MS,et al) and beg for authorization to reinstall ....
