I'm having the same issue on a brand new t440s, and the hiss stops when muting the sound. Happens both on speakers and headset.
This issue is really annoying when listening to music with quitet periods in it.

My sound started out intermittment and is now persistent, especially when plugged in. So there is no dedicated solution to this problem? And it is DEFINITELY hardware? But what if I send it into hardware and they have to just replace it again, because they don't have a dedicated solution?

i just recevied a brand new T550 (16 GB Ram, I7, LTE, SCR, FPR, 1 TB SSD Sandisk Extreme, 3k non Touch, Precisioon Keyboard, Windows 8.1 recently installed all new drivers from lenovo site) - i upgraded the disk on arrival of the notebook.
and.... i also have this issue. I had the notebook run on Win 7.1 64.bit over the weekend (it had the issue there as well) then upgraded to win 8.1 today, still same issue. Currently i only have windows standard drivers installed, the current realtek drivers are just downloading. Will post a reply when installed.
The noise is pretty much annoying as the notebook is very quiet...

Update: Installed drivers from lenovo (Version 220.127.116.1183) page did not fix the issue! The noise starts and lasts around 4 seconds after the initial audio being played (like windows system sounds)
