TL;DR it has to be 2R RAM.
Hi guys. T400 here. It's a RAM thing.
2x HMT125S6TFR8C-G7 which is DDR3 2GB 1066MHz 1.5V CL7 2R
2x KTH-X3CL/4G which is DDR3 4GB 1600MHz 1.35V CL11 1R
Put back the old RAM and everything works fine.
It's a "R" thing. More about that was written here by Tasurinchi.

I just had this problem. (T420 - 4177CTO)
For me the fix was to remove and reseat the DIMM's.
I'm running 16G total - 8G under the keyboard and 8G in the slot on the underside of the computer.
