I think you could be right, after looking at other people's problems regarding fans, there seems to be cases where there are premature failure of the Toshiba sourced Fan assembly. You should send it back to depot and ask it to be looked at.

Same thing here. I have a T61 that's about 9 months old. And about a month ago i started hearing a weird grating/buzzing  noise that i first thought was coming from by hard drive. Then realized it was coming from the fan area in the back not the front. And there it is now.  Anyways....
              I regularly clean my TP out with compressed air and do so in detail when ever i open it,  there is usually a dust ball of two every time.You would be surprised how quickly dust can accumulate in computers, Especially when their just sitting on a desk.  I thinkl its because of static and magnetic Fields they create.  Pulls it right out of the air. And when it dose it can cause shorts or even disrupt ram chips so badly the computer wont recognize them until you've cleaned and reseeded them. 
 
The good news....
 Its a simple replacement  fix covered under warranty and is not an Uncommon problem (For a Older computer anyways).
    I've had a lot of exspirence with computer maitinance and it sounds just like a hard drive going bad. like it has a moter going bad. No amount of cleaning seems to help. The odd Part is occures at weird intervals and not only when the fan is working harder.  Im Just waiting for a good time to send it in for service.  .
 
The bad news...
    While Im sure this was probubly a bad shipment from toshiba not lenovo's fault. similar problem seem to be occuring with alot of other items on with T6x's Their are alot of T61's with a flaw in the mother board that cause the useport on the right side to be underpowered so that things like usb hard drives and pda chargers .etc.. things that require usb 2.0 to funtion properly.  causing problems like the computer not recognizing a device,  not charging your pda. or requiering that removable devices be reinstalled everytime it is pluged in. incomplete syinkin of devices. the usb ports on the left are un affected so for most people its not a big deal unless your already using those ports or you plug a wirless mouse reciver in on that side and you right handed.

dont get me wrong i love my T61. and i just ordered my second Lenovo thinkpad. but It makes that 3-4 yeat exstended service plan a very good idea.

The high pitched noises are due mostly to accumulated dusts and lack of lubrication of the bearing. So after cleaning the fan assembly with compressed air the noise remain, you should contact service and ask it to be looked at, prevention is better than cure. Also if your fan fails after warranty, then you probably are going to be hit with a 200+ USD maintenance bill, these heat pipe and fan heat sinks are mighty expensive. Even a second hand one off ebay will set you back around 50 USD with additional charge for shipments, so you can imagine how much that is going to cost new from depot plus labour and shipping charge.

The high-pitched noises are very annoying...  My T61 was silent as a mouse when I first got it.  Once I started installing updates, things went bad.  I'm not absolutely sure, but I may have found a correlation to the video driver in my image (my T61 has a corporate XP image and Intel graphics).  The image drivers (I think) were 6.14.10.4860.  I updated to 6.14.10.4926 via System Update.
 
It was some time after this point that the noise started for me.  I rolled back my driver to see, and the noise got a little better... I think.  I will have to try it again.  I noticed while I had it rolled back that I could turn off the power saving features of the video driver, so I turned them off, and reinstalled the .4926 driver.  The noise came back... it comes and goes.  I think I'll try to roll back again to recheck [Does anyone have the Lenovo download for 6.14.10.4860?]
 
I started this whole goose chase when I noticed that the sound went away when I brought a WIMP window to the foreground.  Now it could have been CPU related, but the screen had lots of visualizations being rendered.
 
I'll also get a can of compressed air and try to clean out the insides.  Since this is the second T61 that this happened to, I'm more inclined to thing it is related to a driver problem.

Hello, reporting back :)

I did the dust cleaning job week ago and the problem still remain.
So i take the laptop to the nearest thinkpad service center to claim my warranty.
Fortunately my laptop are still in warranty period.

I waited for 10 days and finally today i got the call.
The new fan have arrived!
It took about an hour for the technician to install the new fan and finally my T61 was silent again :)
This cost me no money at all because of the warranty.

$200 maintenance bill for the fan?!
wow! veery expeeensive... What if this new fan fails? -__-;
I wonder if we can change just the fan, maybe it could be cheaper?

Oh, fyi, i bought this laptop on May 2007.
So it took my fan about  13 months to fail.
And i use my T61 as my primary computer, imagine ~12 hour a day for almost everyday in one week in a room without air conditioner... Maybe i stressed it too much :P

The fan especially in high dust and humid environment may cause the fan to fail prematurely due to dust build up and use up of lubrication. If you can add some lubrication to the bearing of the fan, then this problem maybe fixed.
