I'm up to date on all updates available through Lenovo Vantage and all previous Windows Updates. Trying to get Windows 1903 so can use the PC XBOX app. Even though on the Lenovo site is says the X1 Extreme is compatible with Windows 1903, going to windows update gives me this message about 1903:
The Windows 10 May 2019 Update is on its way. We're offering this update to compatible devices, but your device isn't quite ready for it. Once your device is ready, you'll see the update available on this page. There's nothing you need to do at this time.
What else do I need to do to make my maxed out X1 Extreme eligible for this update (still under warranty also).

Are you sure that statement is accurate? From Microsoft's website:
Current status as of June 18, 2019:

1. Please Go to Settings->Update&Security—>Windows Update. Run Windows update , make sure all 1809 package is up to date . Maybe need more times run Windows update in here.
2. If 1903 still not avaliable . Please download upgrade tool to upgrade system manually if you need. .URL:https://www.microsoft.com/en-us/software-download/windows10

I have the same problem with my X1 Extreme. I have a fully updated Windows 10 1809 (via Lenovo Vantage and Windows Updates).
The message translates to: my device is currently not compatible with 1903. (And I do not have any USB sticks inserted, which could be a source of trouble..)

All of a sudden today Windows 10 version 1903 update started showing as available to download and install for me even though I was getting that incompatability message for the past couple of weeks. I haven't changed anything with my system so I assume it's a Microsoft thing,

I am not sure if this will help anyone else but it is what made 1903 available for me. I am NOT on a X1 extreme but am on a X1 Carbon. Same exact situation, same messages, made sure EVERY update was installed and still no luck. Searched some forums and read that ANY installed USB drive or SD card installed will stop the update. I thought it sounded crazy but removed the 32 GB micro SD card I keep installed for backups and was INSTANTLY able to install the update. Did entire update, reinstalled my micro SD card and all is well.
I could not believe that was the issue but I know for certain that was the hold up.
Hope it helps!
