Out of the blue fans started spinning fast and producing lots of noise.
I waited for about an hour but they continued with no sign of improvement.
Apparently it happened to this server on a few occasions in the past.
It runs ESXi 6.0.0 and appeared to be functioning ok together with all hosted VMs.
I have found the following errors:
Power Module (DC-to-DC) 1 DIMM AB VR --- Lower Critical --- 0 Volts
Power Module (DC-to-DC) 1 DIMM CD VR --- Lower Critical --- 0 Volts
Power Module (DC-to-DC) 1 DIMM EF VR --- Lower Critical --- 0 Volts
Power Module (DC-to-DC) 1 DIMM HG VR --- Lower Critical --- 0 Volts
More strange readings from Sensor Monitoring:
CPU Usage 0%
CPU Power 0 Watts
DIMM Power 0 Watts
Firmware versions are reporting as below:
BIOS LG_BIOS_000 1.7.0
Deployment Manager LG_TDM 1.0.3
Linux Driver Bundle LG_LIND 1.0.0
System Manager LG_TSM 1.10.61592
Windows Driver Bundle LG_WIND 1.0.0
I shut the server down, removed power cords and reseated PSUs.
After several minutes I reconnected power.
There was an amber warning on the front panel which went away after a few minutes.
Following that the server and all VMs started up fine.
All readings in logs and Sensor Monitoring now look normal and the fan noise has gone away.
Has anybody reported a similar problem before?
What's the cause and How to prevent it from happening again?

This problem was caused by BMC didn't working. System BIOS and BMC version are too lower, please update the latest BIOS and BMC version, system will work well with the latest BIOS and BMC.

How about updating LG_TDM and LG_LIND (ESXi is Linux based)?
I'm guessing no harm doing LG_WIND in case server is repurposed in the future, right?
What's the recommended update procedure, order etc?

Please also update latest LG_TDM, https://download.lenovo.com/pccbbs/thinkservers/tdm_v1219.exe. LG_LIND,https://download.lenovo.com/pccbbs/thinkservers/tdm-linux-drivers_v1219.exe. LG_WIND,https://download.lenovo.com/pccbbs/thinkservers/tdm-windows-drivers_v1219.exe.
Recommende the update order as BMC->BIOS->TDM.

So the ultimate order is:
BMC -> BIOS -> TDM -> LG_LIND -> LG_WIND
using Lenovo ThinkServer System Manager Web GUI -> Firmware Update -> Upload a firmware bundle
